Getting Ready for Your Visit

We want to make your visit as easy and timely as possible. With new federal privacy regulations in place, there may be paper work that needs to be completed prior to your visit. In order to make things easier for you, we have included many of the required forms for you online allowing you the ability to print them out and complete them prior to your visit saving you time.

In addition to necessary office forms, it is important that you make sure that you have obtained the proper referrals prior to your visit. This is very important in that without the proper referral from your primary doctor, your insurance company may not cover your visit with us.

We have compiled a check list for you to help get everything ready.

New Patients

To our new patients, welcome! We look forward to meeting you.

Here is what you’ll need to make your visit with us run smoothly:

1. A completed new patient form, medical information form, and a medical service coverage acknowledgement form (these can be filled out in our office, or you may print them and fill them out prior to your visit. To view and print a copy of all three forms,click here).

2. Please review our Notice of Privacy Practices and complete the Acknowledgement of Receipt of Privacy Policies form (this too can be viewed and filled out in the office or completed prior to your visit.  To print and complete the form at home, click here.)

3. If your insurance company requires it, make sure that you have the proper referrals from your primary doctor PRIOR to your visit with us.

4. Please make a list of your current medications and the names of medications you have tried for your skin condition prior to your visit with us. This will help us greatly to give you safest and most accurate treatment.

5. Please bring a copy of your insurance card with you to your visit.

Established Patients

To our established patients, we look forward to seeing you again.

If you have not been seen in our office for over 3 years, please refer to the “New Patients” section above to prepare for your visit.
